Welcome to Crewkerne Health Centre

Full_tree_chcWe hope this website will prove useful both to new patients and to patients who may have been registered with us for many years. We aim to provide a comprehensive and professional service and we hope the following information will enable you to make full use of the Health Centre.

The building has disabled access and facilities, including a hearing loop.

The Health Centre is adjacent to Crewkerne Community Hospital and we provide health care for patients living in Crewkerne and the surrounding villages within a five mile radius.

The doctors, practice nurses, counsellors and administrative staff work in conjunction with the district nurses, midwives, health visitors, school nurses and auxiliary nurses to form a Primary Health Care team. We believe that by working together within that ‘Team’ we can provide a comprehensive service to our patients in the community. We are committed to the values of traditional General Practice and we want to provide the best possible service.

A new pharmacy will open in the Health Centre in August 2012.  Please read here for further information.

Dr Zwartouw is retiring

Dr Carol Zwartouw is retiring from her postion as a full time partner at the Health Centre at the end of May 2012.  Whilst we will all be sorry to see Dr Zwartouw leave the practice, we are delighted that Dr Susan Davies will be joining us as a full time partner on 11th June. Dr Zwartouw's patient list is being split between Dr Davies and Dr Julia Hewson and we will be writing to all Dr Zwartouw's patients advising them of their new GP.

Extended hours

From 4th July 2011 the times of our extended hours surgeries have changed.  GPs will now hold extended hours surgeries from 6.30 pm - 7.30 pm on Monday evenings.  A family planning clinic will also be held at the same time.

Virtual Patient Group

The practice would like to set up a 'virtual' patient group to work with practice staff to consult on matters which are important to our patients.  Please click the button on the right hand side of the page if you are interested in finding out more.
